Cancer Chemotherapy Using continuous Intera-arterial Methotrexate, and Intermittent Intramuscular Citrovorum Factor

Abstract
This paper reports our experience with 21 patients with advanced cancer who were treated with intraarterial Methotrexate, at the Kwangju Christian Hospital during a two year period, (July 1965-September 1967).
There were eight patients with maxillary cancer, six with cervical cancer, three with stomach cancner, two with hepatoma, and one each with rectal and tongue cancer. Every patient had either an un-resectable lesion, or a post-operative recurrence.
The technique of treatment employed was continuous administration of Methotrexate into the artery or arteries supplying the site of the lesion via a small catheter, along with intermittent intramuscular injection of citrovorum factor.
Therapeutic response, drug dosage and toxicity occurred as follows:
Eleven patients responded with objective reduction in tumor size and with symptomatic relief. The duration of response was from 14 to 210 days, with a mean of 70 days. The dosage at which response began ranged between 90-350 mg. The duration of treatment before response began ranged from 3 to 27 days, (Table 2).
Toxic effects of Methotrexate include bone marrow depression, stmatitis, skin rash, nausea, vomiting, and diarrhea.
Stomatitis was seen in 5 of the eleven patients who responded to treatment, and in three of the ten patients who did not. Leukopenia was more common in those patients who responded to treatment, but leukopenia and thrombocytopenia when it occurred in non-responders were more severe, (Table 3. & 4). A skin rash developed in three patients of the 21, two of whom did not respond to treatment, (Table 4).
The incidence of nausea and vomiting was about equal in both groups.
